Access the KCA Sycamore Online Re-enrollment Portal.

Submit the required forms to complete the re-enrollment process.

  • Annual Physician's Statement Form (18 months - 4 years)
  • Health Statement Form (Grades K - 12)
  • KCA Re-enrollment Agreement
